The EOI is for a baseline data collection (currently planned for Nov. and Dec. 2015) for the CPBF IE, as part of a WB-supported Maternal and Child Health and Nutrition Project.

The survey includes 8 districts in 4 regions: Northern: North Gonja, West Gonja; Upper East: Bawku West, Talensi; Upper West: Lawra, Nandom; Volta: Agortime Ziope, Kadjebi

Data to be collected (approx. samples in brackets): Surveys of Communities (150), Households (3000), Community health teams (150*ca.4 staff), CHPS Compounds/Health centers (150), Regional management (4), District health management (8) & Patient exit interviews (community health team level) (2000)

The firm is responsible for the logistics & ensuring high quality data, incl:
-Assisting with the instrument finalization including conducting survey piloting
-Procurement of survey-related consumables/equipment
-Enumerator training
-Producing a field procedure plan
-Collect the data incl. anthropometric measures & blood samples
-Reporting
